Palliative Care

Our Palliative Care service, working in partnership with Farleigh Hospice, is designed to ensure that all “end of life” service users are treated with dignity and respect. All care staff assigned will be committed to providing adequate support as well as reassurance that each person is being treated as an individual.

Sensitivity will be given by coordinator teams in order to provide continuity of care wherever possible.
Coming to terms with a life-threatening illness can be extremely difficult for both you and your family. Our palliative care service aims to offer support to you and your family during this difficult time whilst enabling you to receive the best possible care in the comfort of your own home.

You will be provided with a personalised care plan that will take into account your needs, preferences and choices. Every service user will be assigned a key worker who will report back to the Care Manager as your health changes. The key worker will liaise with District Nurse Teams, family and friends for and on your behalf emphasising and ensuring your wishes are met as necessary.

Caring Direct offer on-going support during what can be a difficult and upsetting time in order to relieve as much stress from other family members also. Care can be suspended during admission periods into hospital and resumed upon return home. Care staff can be present to share in your good news and comfort you when you receive bad news.

As your care requirements increase, your key worker will discuss the possibility of additional care to suit your needs.
